Pulpitis is inflammation of the tooth pulp, the soft tissue containing nerves, blood vessels, and connective tissue. It is one of the most painful dental diseases that requires immediate medical attention. Causes of Pulpitis Development

The main cause of pulpitis is untreated caries. When the carious process penetrates through the enamel and dentin into the pulp chamber, bacteria infect the pulp, causing its inflammation. Our specialists note that the deeper the caries, the higher the risk of developing pulpitis. Many patients postpone caries treatment due to the absence of pain in the early stages, but when the infection reaches the pulp, acute pain occurs, requiring urgent intervention.

In addition to caries, in our clinic we observe pulpitis that occurred as a result of tooth trauma – crown fracture with pulp exposure, root fracture, tooth dislocation. Also, pulpitis can develop after improper dental treatment – tooth overheating during preparation without water cooling, accidental opening of the pulp chamber. Sometimes pulp inflammation occurs under an old filling due to the development of secondary caries or violation of restoration tightness.

Types of Pulpitis and Their Manifestations

In our dentistry, we distinguish several forms of pulpitis, each of which has its own course characteristics and requires a certain approach to treatment. Acute focal pulpitis is characterized by localized inflammation near the carious cavity. The patient experiences paroxysmal acute pain that occurs spontaneously, especially at night. The pain intensifies from cold, hot, sweet and continues for some time after the irritant is removed.

Acute diffuse pulpitis develops when inflammation spreads to the entire pulp. The pain becomes more intense, throbbing, radiates to the temple, ear, adjacent teeth. Painless intervals are shortened, pain can be almost constant. Our doctors often observe that patients cannot accurately identify the aching tooth, as the pain spreads throughout the jaw. Chronic pulpitis proceeds with less pronounced symptoms – periodic aching pain, discomfort when chewing, sensitivity to temperature stimuli.

Pulpitis Diagnosis

Pulpitis diagnosis in our clinic begins with a thorough patient interview. Our specialists clarify the nature of the pain, when it appeared, what provokes attacks, how long the pain continues after the irritant is removed. This data helps us differentiate pulpitis from other diseases with similar symptoms – deep caries, periodontitis, neuralgia.

During examination, our doctors discover a deep carious cavity, probing the bottom of which is sharply painful. We perform tooth percussion – with pulpitis, tapping is usually painless or slightly painful, which distinguishes it from periodontitis. The thermal test shows a pronounced reaction to cold and especially hot. We necessarily perform a radiological examination, which allows us to assess the depth of carious damage, the condition of the roots, the presence of periapical changes.

Methods of Pulpitis Treatment

In our dentistry, two main methods of pulpitis treatment are used – biological (conservative) and surgical. The biological method is aimed at preserving pulp viability and is used in rare cases of focal pulpitis in young patients, when no more than 1-2 days have passed since the onset of symptoms. Our specialists apply a medicinal lining with calcium hydroxide to the exposed pulp, which relieves inflammation and stimulates the formation of a protective dentin layer.

However, in most cases in our clinic, the surgical method is used – complete removal of the inflamed pulp from the crown part and root canals. This is the most reliable treatment method that guarantees pain relief and prevents the development of complications. We perform pulp removal, thoroughly process and fill the root canals, restore the crown part of the tooth.

Stages of Surgical Treatment

Pulpitis treatment in our clinic begins with quality anesthesia. We use modern latest generation anesthetics that provide deep and long-lasting anesthesia. In acute pulpitis, sometimes enhanced anesthesia or additional injections are required, as the inflamed pulp is less sensitive to anesthetics. Our specialists always achieve complete anesthesia before starting treatment.

After the onset of anesthesia, we isolate the tooth with a rubber dam to ensure sterility. Then our doctors remove all carious tissues, create access to the pulp chamber, open it, remove the crown pulp. Using endodontic instruments, we extract the pulp from the root canals, perform their mechanical processing, expand and give them a conical shape. Intensive medicinal treatment with antiseptic solutions is performed in parallel.

Features of Acute Pulpitis Treatment

When treating acute pulpitis in our dentistry, we often use a two-stage method. At the first visit, our specialists open the pulp chamber, remove the crown pulp, which ensures the outflow of inflammatory exudate and quickly relieves pain. A devitalizing paste is placed in the root canals, which ensures the death of the pulp in the canals, a temporary filling is installed.

After a few days, the patient returns to our clinic to complete treatment. We remove the temporary filling and devitalizing paste, extract the remnants of pulp from the canals, perform their final mechanical and medicinal treatment, fill the canals with permanent material, restore the crown part of the tooth. This approach allows for more comfortable treatment for the patient and minimizes the risk of postoperative complications.

Treatment of Chronic Pulpitis

Chronic pulpitis in our clinic is most often treated in one visit, as there is no acute inflammation. Our specialists perform complete pulp removal, thorough processing of root canals and their filling in one visit. We use modern rotary nickel-titanium instruments that ensure fast and quality processing of canals of any curvature.

In fibrous pulpitis, when connective tissue growths predominate in the pulp, we use the standard pulp removal technique. Gangrenous pulpitis, in which pulp necrosis occurs with an unpleasant putrid smell, requires particularly thorough medicinal treatment. Our doctors use powerful antiseptics, ultrasonic activation of irrigation solutions, temporary filling with antiseptic paste for complete disinfection of the root canal system.

Root Canal Filling

After pulp removal and canal treatment in our dentistry, their hermetic filling is performed. We use the lateral condensation technique of gutta-percha – a main gutta-percha point corresponding to the size of the last instrument with which the canal was processed is inserted into the canal. Then our specialists insert additional points with endosealant, compacting them laterally until the canal is completely filled.

Also in our clinic, the technique of vertical condensation of thermoplasticized gutta-percha is used. Heated gutta-percha becomes fluid and fills all branches, deltas, additional canals, providing three-dimensional obturation of the root system. We necessarily take a control X-ray after canal filling to ensure the quality of obturation – the canals should be filled for their entire length, to the root apex, without voids and perforations.

Tooth Restoration After Treatment

After root canal filling in our clinic, it is necessary to restore the crown part of the tooth. We choose the restoration method depending on the degree of destruction. With a small defect, our specialists install an aesthetic composite filling, restoring the anatomical form and tooth color. We use a multi-layer composite application technique to achieve a natural-looking restoration.

If the tooth is destroyed by more than half, we recommend installing a ceramic crown. A depulped tooth becomes fragile, as its nutrition from within ceases, the walls can thin, the risk of fracture increases. The crown protects the tooth from destruction and ensures its durability. With significant destruction, our doctors first strengthen the tooth with a post and core or fiberglass post.

Possible Complications and Their Prevention

In our dentistry, we make every effort to prevent complications after pulpitis treatment. However, patients should know about possible situations. After treatment, pain when biting on the tooth may persist for 2-3 days – this is a normal reaction of periodontal tissues to instrumental canal processing. Our specialists warn about this and recommend taking pain relievers if necessary.

We advise patients to immediately contact our clinic if the pain does not decrease but intensifies, swelling of the gum or cheek appears, body temperature rises. This may indicate the development of complications – removal of infected material beyond the root apex, incomplete pulp removal, root perforation. The best way to avoid pulpitis is timely caries treatment. At this stage, treatment is fast, painless and inexpensive. If caries penetrates deeply, the risk of developing pulpitis significantly increases.

The basis of prevention is quality oral hygiene. We teach our patients the correct teeth brushing technique, recommend suitable hygiene products. It is necessary to brush teeth at least twice a day, use dental floss or an irrigator to clean interdental spaces. Is it possible to treat pulpitis without nerve removal?

Theoretically, with very early presentation at the stage of acute focal pulpitis, conservative treatment with preservation of pulp viability is possible. However, in our clinic we use this method extremely rarely, as it has a high risk of failure. Most often, patients present with pronounced inflammation, when conservative treatment is ineffective. Our specialists recommend complete removal of the inflamed pulp as the most reliable method that guarantees pain relief and prevents disease recurrence.

How long does pulpitis treatment take?

The duration of pulpitis treatment in our dentistry depends on the complexity of the case and the chosen technique. With one-stage treatment, all procedures are performed in one visit lasting 1.5-2.5 hours depending on the number of canals. Multi-rooted teeth require more time. With two-stage treatment, the first visit is usually shorter – 30-60 minutes for pulp devitalization, the second visit takes 1.5-2 hours for final canal processing and filling.

Why does the tooth hurt after pulpitis treatment?

Slight pain for 2-3 days after pulpitis treatment is a normal reaction. In our clinic, we explain to patients that during treatment, periodontal tissues are traumatized by instruments, antiseptics, and filling material. The body needs time to adapt and heal. The pain should gradually decrease. If the pain intensifies, becomes throbbing, swelling appears, it is necessary to urgently contact our clinic to identify the cause and correct treatment.

Is it necessary to install a crown after pulpitis treatment?

The need for crown installation in our dentistry is determined by the degree of tooth destruction. If more than half of the crown part is preserved and the tooth walls are thick enough, our specialists restore the tooth with a composite filling. With destruction of more than 50% of the crown, we strongly recommend installing a ceramic crown. A depulped tooth loses nutrition, becomes fragile, there is a high risk of fracture under chewing load. The crown protects the tooth and ensures its durability for many years.
