Tooth Nerve Removal and Canal Cleaning in Vilnius: Endodontic Treatment

Tooth nerve removal, or pulp removal, is a dental procedure in which the tooth pulp containing nerves and blood vessels is removed. In our clinic, endodontic treatment is carried out using modern technologies and materials, which allows us to preserve the tooth and relieve the patient of pain. The pulp is located inside the tooth in a special chamber and root canals, and when it becomes inflamed or infected, severe pain occurs, requiring immediate treatment.

Our specialists perform root canal treatment with quality and painlessly, using modern equipment and microscopes to control each stage of the procedure. We understand that patients often feel anxious before this procedure, so we create comfortable conditions, use effective anesthesia, and explain each stage of treatment in detail.

When Nerve Removal is Necessary

In our clinic, we recommend tooth nerve removal in several situations. The main indication is pulpitis, inflammation of the pulp, which is most often a complication of deep caries. When the carious process reaches the pulp chamber, bacteria infect the pulp, causing its inflammation. The patient experiences acute throbbing pain that intensifies at night, when exposed to cold or hot.

We perform pulp removal for periodontitis – inflammation of the tissues around the root apex. This disease develops as a complication of untreated pulpitis, when the infection spreads beyond the tooth. Also, our specialists remove the nerve in case of tooth trauma, when a crown fracture occurred with pulp exposure, or when preparing the tooth for prosthetics if significant grinding is required. In some cases, pulp removal is necessary before installing a crown on a severely damaged tooth.

Symptoms of Pulp Inflammation

Pulp inflammation manifests with characteristic symptoms, upon the appearance of which we recommend our patients immediately contact our clinic. The main sign of pulpitis is acute spontaneous pain that occurs without apparent reason and intensifies in the evening and at night. The pain can be throbbing, shooting, radiating to the ear, temple, adjacent teeth.

A strong reaction to temperature stimuli is characteristic – cold and especially hot. Moreover, the pain does not go away immediately after removing the irritant but continues for a long time. In acute pulpitis, general symptoms may be observed – body temperature rise, weakness, sleep disturbance due to pain. Our doctors conduct thorough diagnostics to determine the cause of pain and choose the optimal treatment tactics.

Diagnosis Before Treatment

Before tooth nerve removal in our dentistry, comprehensive diagnostics are performed. Our specialists begin with an examination of the oral cavity, interviewing the patient about the nature of pain and duration of symptoms. We perform tooth percussion – tapping on it to determine pain, palpation of surrounding tissues, thermal test to assess the tooth's reaction to temperature stimuli.

A mandatory diagnostic stage in our clinic is radiological examination. We take a targeted X-ray of the affected tooth or a panoramic X-ray of all teeth. X-rays allow us to assess the condition of root canals, identify hidden carious cavities, determine the presence of inflammation at the root apex, cysts or granulomas. Based on the obtained data, our doctors create a treatment plan and determine the prognosis.

Stages of Endodontic Treatment

Root canal treatment in our clinic is carried out in several stages. The procedure begins with quality anesthesia – we use modern anesthetics that completely eliminate pain during treatment. After the onset of anesthesia, our specialists isolate the tooth using a rubber dam – a special latex curtain that protects the tooth from saliva and ensures sterility of the working field.

The next stage is creating access to the pulp chamber. We form a cavity in the tooth crown through which we gain access to the root canal orifices. Then our doctors remove the inflamed or infected pulp from the pulp chamber and root canals using special endodontic instruments. We use both hand files and modern machine nickel-titanium instruments that ensure effective and safe canal processing.

Mechanical and Medicinal Processing

After pulp removal, our specialists perform thorough mechanical processing of root canals. We expand the canals using endodontic files of increasing diameter, give them a conical shape, which is necessary for quality filling. Our clinic uses modern rotary nickel-titanium instruments that have high flexibility and effectively process even curved canals.

Parallel to mechanical processing, we perform intensive medicinal canal treatment. Our doctors irrigate the canals with antiseptic solutions – sodium hypochlorite, chlorhexidine, EDTA. These solutions destroy bacteria, dissolve organic residues, remove dentin filings. We use ultrasonic activation of antiseptics, which significantly increases the effectiveness of root canal disinfection and improves treatment prognosis.

Determining Root Canal Length

Accurate determination of root canal length is critically important for successful treatment in our clinic. If the canal is not filled to the end, bacteria multiply in the remaining space, leading to inflammation. If the material is removed beyond the root apex, it can cause pain and complications. Our specialists use an apex locator – an electronic device that determines the working length of the canal with high accuracy.

Additionally, we take control X-rays with files inserted into the canals, which allows us to visually assess the depth of the instrument and adjust the working length. In our dentistry, a dental microscope is used, which provides multiple magnification and allows the doctor to see canal orifices, control the processing process, identify additional canals that can be missed when working without magnification.

Temporary and Permanent Canal Filling

Depending on the clinical situation, treatment in our clinic can be performed in one or several visits. In case of acute inflammation, pus in canals or periapical tissues, we use two-stage treatment. After mechanical and medicinal processing, our specialists place medicinal paste with antiseptics and anti-inflammatory components in the canals, install a temporary filling.

After 5-7 days, if the tooth does not bother the patient and the inflammation has subsided, we perform permanent root canal filling. Our doctors use the lateral condensation technique of gutta-percha – the canal is filled with a main gutta-percha point and additional points with endosealant. We also use the technique of vertical condensation of heated gutta-percha, which ensures three-dimensional canal obturation, filling of all branches and deltas.

Restoration of Crown Part of Tooth

After root canal filling, it is necessary to restore the crown part of the tooth. In our clinic, we use various methods depending on the degree of destruction. If less than half of the crown is destroyed, our specialists restore the tooth with a composite filling. We create an aesthetic restoration that returns the tooth to its anatomical form and functionality.

With significant tooth destruction, when less than 50% of crown part tissues remain, we recommend installing a ceramic crown. A depulped tooth becomes more fragile as it does not receive nutrition, and to prevent its fracture, reinforcement with a crown is necessary. In cases where the tooth walls are very thin, our doctors first install a post and core or fiberglass post to strengthen the structure.

Treatment Under Microscope

In our dentistry, we perform endodontic treatment using a dental microscope. This modern equipment provides magnification up to 30 times and excellent illumination of the working area, which allows our specialists to see all the details of root canal anatomy. The microscope helps detect additional canals that are easy to miss when treating without magnification.

The use of a microscope in our clinic significantly increases the quality and prognosis of endodontic treatment. Our doctors can precisely control the canal processing process, remove old filling material during retreatment, extract broken instruments, close root perforations. The microscope is especially important when treating complex cases – curved canals, obliterated canals, teeth with non-standard anatomy.

Possible Complications

In rare cases, complications may arise after tooth nerve removal and canal filling. Our specialists make every effort to prevent them, but patients should know about possible situations. After treatment, pain when biting on the tooth may persist for several days – this is a normal tissue reaction to intervention that goes away on its own.

We warn our patients that it is necessary to contact our clinic if the pain intensifies, swelling of the gum or cheek appears, body temperature rises. This may indicate the development of inflammation. Also possible is the removal of filling material beyond the root apex, which sometimes causes discomfort. In most cases, the body adapts, but with persistent complaints, we perform treatment correction.

Care After Endodontic Treatment

After tooth nerve removal and canal filling in our clinic, we give patients detailed care recommendations. For 2-3 hours after the procedure, while the anesthesia is working, you should not eat to avoid accidentally biting the numb cheek or tongue. In the first days, we recommend chewing on the opposite side, avoiding too hard food that may create excessive load on the treated tooth.

It is necessary to maintain quality oral hygiene, but teeth should be brushed carefully without injuring the gum in the area of the treated tooth. Our specialists may prescribe rinsing with antiseptic solutions, taking anti-inflammatory drugs if necessary. It is important to come for a control examination at the appointed time – we will check the condition of the tooth, if necessary, take a control X-ray to assess the quality of canal filling.

Alternative to Tooth Extraction

Endodontic treatment in our clinic allows us to preserve the tooth, which would otherwise have to be extracted. Many patients ask if it is not easier to remove a diseased tooth than to treat canals. Our specialists always strive to preserve the patient's own teeth, as this has many advantages. The preserved tooth continues to participate in chewing, maintains proper load distribution on the jaw.

After tooth extraction, adjacent teeth begin to shift toward the defect, the bite is disrupted, overload of remaining teeth occurs. Prosthetics will be required to restore the integrity of the dental arch – a bridge prosthesis, implantation, which is significantly more expensive than canal treatment. In our dentistry, we use modern technologies that ensure a high percentage of successful endodontic treatment and long-term tooth preservation.

Frequently Asked Questions

Is it painful to remove tooth nerve?

Tooth nerve removal in our clinic is absolutely painless. We use modern quality anesthesia that completely blocks painful sensations. The anesthetic injection may cause slight discomfort for a few seconds, but then the area loses sensitivity. During the entire procedure – preparation, pulp removal, canal processing and filling – the patient does not feel pain. After the anesthesia wears off, slight pain is possible, which is relieved by ordinary painkillers.

How many visits are required for canal treatment?

In our dentistry, root canal treatment can be performed in one or several visits depending on the clinical situation. For planned pulp removal without acute inflammation, we often perform treatment in one visit – we process the canals and immediately fill them. In case of acute inflammation, pus, periodontitis, 2-3 visits are necessary with temporary filling with medicinal paste between visits. Complex cases may require more visits.

Will the tooth darken after nerve removal?

After nerve removal, the tooth may slightly change shade, become slightly more matte, as blood supply and nutrition of the tooth from within ceases. In our clinic, we use modern technologies that minimize color change. Our specialists thoroughly remove all pulp residues from the pulp chamber, use quality materials for canal filling. If the tooth has darkened significantly, we can perform internal bleaching or install a ceramic crown to restore aesthetics.

How long will a depulped tooth last?

With quality root canal treatment in our clinic and proper restoration of the crown part, a depulped tooth can serve for decades, sometimes for life. Our specialists use modern equipment, microscopes, quality materials, which ensures a high percentage of successful endodontic treatment. It is important to protect the depulped tooth with a crown with significant destruction, regularly visit our clinic for control examinations, maintain quality oral hygiene.
